Add Avalon form widget to Sitefinity page ^

Used for creating stand-alone form pages not necessarily associated with a package.
Log into Sitefinity backend and navigate to Pages tab.
Click on the ‘Create a page’ button
Select a name and choose if you would show the page in the top navigation
Select one of the Custom templates
On the right-hand side from the Content sidebar, click on the Avalon MVC
Drag the “Avalon form” widget and drop it into the Main content section of a page.
On the Avalon form widget click on the Edit button and in the FormId field, enter the id of the form that you want to be displayed on that page. Save
Publish the page
The page can be further styled as desired. A link to the Form page can be send to users via email for instance.
