Create base form

You can either choose to create a form from scratch or clone from an existing form.

Option 1 - Create form

1. Go to UPlay content > Forms.

2. Click tools icon at right-side and click +New. A new window will pop up.

3. At Forms edit screen, input values into respective fields.

Title *	Name of the form.	Textbox
Show form title?	Determines if the form title will be displayed in the form listing or not.	Checkbox
Subtitle	Subtitle or short description of the form purpose - usually displayed below the form title if the field is populated.	Textbox
Details	Purpose or description of the form. The details text can be formatted, images can be added to make the details section more descriptive. If populated, the Details value will be displayed under the form Title and Subtitle.	HTML editor
RX contact *	Name of organiser, will be listed at bottom of a form.	Textbox
RX contact email *	Email of organiser, will be listed at bottom of a form.	Textbox
Show online	Indicator where the form will be made available online. Check the checkbox for yes (show online), otherwise leave the checkbox unchecked.	Checkbox
Submit button text	Custom label for the Submit button can be set. Eg: Submit Form or Click to submit.	Textbox
Confirmation message	Once the form is submitted, customer will be presented with a confirmation/successful submisison message, hence it is important to populate this field. Eg: Thank you for your submission. If field is blank, submitters would see a blank page after submitting their form.	HTML editor
Confirmation email CC list	Recipients to receive a copy of the successful submitted form. Separate multiple recipients with a comma.	Textbox
Confirmation email BCC list	Recipients to receive a copy of the successful submitted form (recipients are not listed in emails). Separate multiple recipients with a comma.	Texbox
Confirmation email	(a) Layout or email contents of confirmation email can be customized for each form, use specific datasets. (b) Email will be sent to indicated recipients. Separate multiple recipients with a comma.	HTML editor
Send email to form respondent?	Indicator whether submitter will receive a copy of confirmation email. Check the checkbox for yes (show online), otherwise leave the checkbox unchecked.	Checkbox

1. Click Save to proceed with changes or Cancel to exit without saving.

2. Once saved, the form will appear in the forms listing.

Option 2 - Clone from existing form

You can choose to clone from an existing form if you are unsure how to.

1. Go to UPlay content > Forms.

2. Input a keyword into Keywords textbox and click Search button

3. At results section, click Tools icon against the preferred form to clone from to expand hidden options.

4. Click Clone button, the edit screen will popup.

1. At AvalonForm Edit screen, the field values on display are as per the selected form you wish to clone from. You can edit field values accordingly.

1. Click Save to proceed with changes or Cancel to exit without saving.

2. Once cloned successfully, below message will appear.

1. The cloned form will appear in the forms listing.
